Players wishing to enroll in the 2009 Academy Fall League must submit the following documents by August 15th. Players enrolling after August 15th will be charged a $25 late enrollment fee. The following information is available as a printable checklist here.
This form can be mailed or faxed to the league office.
You can pay via check or credit card. Credit Card information can be included on your registration form or you can mail a check made payable to Alabama Soccer Academy.
You can mail or fax a copy of your state certified (not hospital issued) birth certificate.
This form is required by US Club Soccer to issue player passes.
