Parental Consent/Medical Form (PDF | WORD)
This form must be completed by a parent/guardian. It does not need to be signed by your physician. We MUST have your form before you can participate in any camp activities. Medical trainers will review all information submitted on these forms and will be available at Registration to discuss any medical conditions or procedures.
Check-In Procedures
Check-in for residential camps is from 1:00-2:45 pm. Commuters may want to plan to arrive after
2:15 pm, since they will not have to move belongings into the dorm. Stations will be set up for you to check-in, leave money in the camp bank, meet with medical trainers if necessary, pay for/pick up camp ball, receive camp t-shirt, and receive/sign-out room key.
If someone other than the parent/guardian checking-in a camper will be checking-out a camper, please inform us at registration of who that person will be.
Please refer to Daily Schedules for more information. (Day | High School | Elite)
Check-out Procedures
Camp ends after the morning games on the last day. Campers will receive their personal evaluation forms from their coach and a brief closing ceremony will be held on the field. Campers will then be transported back to the dorms from the field at approximately 11:30 am. Campers will move their belongings out of their room, turn in their room key and then be free to leave.
Parents are welcome to watch the morning games and check-out at the field. Arrangements will need to be made to move the camper’s belongings out of the dorm before leaving for the fields at 8:30 am. Campers wishing to check-out at the field must turn in their room key at the conclusion of the closing ceremony and are then free to leave.
Team Discount
Teams are encouraged to attend our new High School Team camp session. However, if that session conflicts with your team's summer calendar, your team can attend an Elite session. If a team of 11 or more attend an Elite session, each team member will receive a discount of $25 per player. Please be sure to have all 11 players list the same team name, so the discount can be applied when all 11 applications are received. Rooming assignments must be made by a team coordinator and submitted to the camp office two weeks prior to your session.
